This form is meant to be an accessible alternative to normal channels of communication. FL Hockey highly encourages you to make direct contact with the proper person of elevated authority with the Association (i.e. Coach, Manager, Level Director, Rules and Ethics Committee, or board members where appropriate). Your concern form will be received by the FL Hockey Rules and Ethics Committee and sent to the person who is most appropriate to deal with your concern. This is likely your coach, level director or manager.
