Thank you for making the decision to tryout for an LLT Athletics program!
Please submit the following 4 required forms: Student Information, Emergency/Medical Information, Student-Athlete Contract, and Sports Physical. The form does auto-save your progress and you can edit your responses after submission if you believe you made an error. Uniform information will be ordered AS-IS so edits after the uniform order has been submitted to our supplier will not be accepted.
We pride ourselves here at LLT that we produce a quality product on the field or court. This is a direct result of our commitment to character-building and focus on continuous improvement in the classroom, in the halls, and on the field. We are looking forward to a great season from the students, teachers, coaches, and parents. It truly takes a village and we appreciate everyone's efforts.
A receipt of submission will be sent to the email address provided and this packet can be quickly edited and changed when a new season comes around throughout the year. A sports physical is a REQUIREMENT, but a physical copy can be submitted to the school nurse. It is easier for record-keeping to upload with this form, but we understand it may not be convenient for all.
