We ask the audience of the performance to respond to this survey, whose purpose is to assess the quality of the audience's experience. Answering the survey takes a few minutes. We will not store your e-mail address or any other personal information. As a result, the responses will remain completely anonymous. Among those who have responded to the survey by November 12th, we will draw three (3) gift cards for the National Opera and Ballet. If you wish to participate in the drawing, we will separately ask for your contact information, which we will not, at any point, associate with the answers provided on the survey form.
