Boone Parent Input Form 2024-2025
As we begin to make plans for the 2024-2025 school year, we would like to use your input when making class placement decisions.   The information you provide, along with input from school staff members, will assist us in placing your child appropriately.  Many variables contribute to the successful placement of students into classes. However, requests for specific teachers will not be honored.  

Please do not name specific teachers on this form. Classroom teachers will be helping to build the class lists in collaboration with essential areas team, administration, and additional campus departments.  Class assignments will be available for students and families at Meet the Teacher in August before school starts.

Items to consider when completing this form:
         List teacher traits and characteristics that support your child.
         What was your child’s best year and why?
         List classroom environmental factors that support your child.

Please complete this electronic form or return a paper copy (one will be sent home soon) by Friday, April 19, 2024.
