Thank you for taking the time to fill out this Survey. Assuming the campus will open in September, it is our hope that all enrolled students will attend in person when their grade is on campus. This form allows us to know if you are comfortable with your child coming to campus.

Important: Because of the nature of questions we are asking, we cannot review answers as "anonymous entries." Therefore: Please do not discuss specific medical or other private information on this form. We will follow up with families who express concerns to discuss options. A space is provided at the bottom of the form for you to provide the best contact number(s) for you.
