Informed Consent
You are invited to participate in a research study about teacher rationales for their policies regarding extensive reading assignments. The goal of this research study is to learn about the breadth of extensive reading assignment grading policies and how those policy choices are influenced by teacher philosophies and / or pressures from the teaching context.

This study is being conducted by Andre Parsons at Hokkaido University of Education, and Patrick Conaway at Shokei Gakuin University.

One qualification to participate in this study is that you currently, or in the past have used extensive reading assignments with your students for language teaching. If you are not involved in directly teaching students, but are involved in the administration of an ER program, we would greatly appreciate it if you could pass this survey on to your teachers.

Participation in this study is voluntary. If you agree to participate, you will answer questions about your teaching context, details of your extensive reading assignments, and reasoning for the various aspects of your assignment choices. Completing the questionnaire is estimated to take from 15 to 25 minutes. We thank you for your valuable time.

Participating in this study may not benefit you directly, but it will help us learn more about the variety of ways that teachers assign extensive reading assignments. Also, by learning about your rationales, it may be possible to make informed choices when designing extensive reading assignments, and better match them to your teaching context.

You may skip any questions you don’t want to answer and you may end the questionnaire at any time.

The information you will share with us if you participate in this study will be kept completely confidential to the full extent of the law.

Your information will be assigned a code number that is unique to this study.

Study findings will be presented only in summary form and your name would not be used in any report. While the investigator(s) will keep your information confidential, there are some risks of data breeches when sending information over the internet that are beyond the control of the investigators.
