MPCS Summer Camp: Friday One Day Only for VBS Campers
Parents: Complete this form and pre-pay $45 Fee via the yellow Paypal.  Includes FREE lunch!

One Day Only:  June 19, 2020.  Friday Only. Cost= $45.  Starts  at 9:00am and Ends 3:30pm.

Level I:  Grades Rising K- 4th Grade.  (Lower School Campus/ White Building. 9820 Nesbit Ferry Rd.)   Drop Off and Pick up at South Hall at 3:30pm.

Level II: Grades Rising 5th Grade - 6th. (Middle School Campus/ Red Brick Building. 9875 Nesbit Ferry Rd ) Drop Off and Pick up at MS Front Lobby at 3:30pm.

Your child will then be in the care of MPCS Teachers and Counselors.  Wear summer camp clothes with sneakers and don't forget your WATER BOTTLE!

I understand if my child is not currently enrolled in Mount Pisgah Christian School as a student, I need to send proof of medical insurance to MPAA@mountpisgahschool.org.  This should be a copy of your insurance card front and back that will be put on file in case of emergency. *

In case of a medical emergency, MPCS Summer Camp will call you first and your emergency phone number.  However, emergency medical care may need to be given and we are unable to reach you.  Thus, we need you to give consent to emergency medical care.  **Emergency Medical Care**:  I give permission for my child to go with 911 emergency first-responders in case of serious accident or  health emergency of my child and I am aware that I accountable for payment to hospital/ emergency services rendered. *

PERMISSION TO TRANSPORT : I, the undersigned parent and/or guardian of a minor, hereby give my permission for my child to be transported to and from Mount Pisgah UMC and MPCS via bus with MP UMC VBS and MPCS.  I do hereby release Mount Pisgah Christian School and Mount Pisgah UMC from any liability and all adult sponsors or camp/ teaching staff in the event of any accident in route, during and returning from MPCS  sponsored events.   *Please add your electronic signature so we can pick up your child after VBS at MP UMC VBS on the big bus and drive them to MPCS which is across the street. *
