2023 / 2024 RSU #14 Laptop Insurance Coverage 
If there are any damages/scratches/dents when your child receives their equipment, please document it and bring it to the schools attention.  As a reminder, all unpaid fees are carried over from one year to the next. Payment is expected prior to high school graduation or your student may not receive a graduation diploma. 

Unpaid balances could be forwarded to a collection agency for continued collection efforts.
This form needs to be completed  prior to student being issued a computer.

We elect to participate in the RSU #14 MacBook Air Insurance Coverage program. We
understand that we are responsible for the proper treatment of the MacBook Air issued to our
child. By taking advantage of this insurance opportunity, we are limiting the financial liability if
the MacBook Air is damaged or is reported missing and not covered under the warranty provided
by the State. Financial liability would be limited to the amount of: $40 insurance fee plus a
deductible amounting to $50 per incident; for families qualifying for free or reduced lunch, the
insurance premium is $6 and the deductible is $50 per incident.
We elect not to participate in the RSU #14 MacBook Air Insurance Coverage program and we accept all responsibility for the MacBook Air assigned to our student(s).  If a family chooses not to insure the MacBook Air, then in the event that there is damage or the MacBook Air is stolen or lost, the family will be responsible for the full cost to repair or replace
the unit.
