This survey is to collect information and parent/guardian input on the return to school. This information is confidential and will only be reviewed by district staff in order to gather the necessary information for planning purposes. No identifiable information will be published. Based upon current COVID health and safety, Jamestown School District is planning for two options for students to return to school. Please select one of the two choices that best represents how you prefer your child to receive instruction this fall. *

All classes conducted remotely through online distance learning

Combination of in-person and online distance learning ( This is a hybrid model with A/B days and one day of all distance learning)

During COVID-19 we are encouraging families to try and transport their children to and from school, if possible. If the district chooses to use a hybrid model on the days that your child attends in-person school , how will your child(ren) get to and from school? *

If there is a decision to have a combination of in-person and online distance learning, any in-person instruction will require that all students and staff will have their temperature taken, will be required to socially distance, and all staff and students 3rd grade and above are mandated to wear face coverings and will remain in small classroom groups or cohorts. (Younger school age children will be strongly encouraged, but not required to wear face coverings). Do you agree to these safety procedures and will send your child(ren) to school? *
