Clinic Fee:
$50 - Includes doctor visit and upload assistance

State Fees:

$104.30 if you do not have Medicare, Medicaid, or SoonerCare

$22.50 with proof of Medicare, Medicaid, or SoonerCare

The State typically takes 7-14 Business Days to process application and Medical Card is sent via USPS
