I hereby certify that I have been vaccinated against COVID-19 and that it has been at least two weeks since my most recent dose.

I further certify that I will continue to follow all event guidelines related to COVID-19, including wearing a mask properly, and washing my hands regularly, even if I have been fully vaccinated.

I understand that providing false information in this certification is a violation of the PyTexas Code of Conduct and may put others at risk. Violating the Code of Conduct can result in expulsion from the conference without a refund. Consequences will be decided at the PyTexas Organizers' discretion.
