Healthy Living Expo - 2024 Vendor Form
The Lynnfield Health Expo Committee reserves the right to choose all vendors. All vendors must abide by all event rules and guidelines for use of town parks. All vendors will be notified if their applications have or have not been accepted.

Vendors receive 1 table, 2 chairs and a 10x10 space.  We do not provide tents.

(Note: Power is limited outside and will be provided in order of registrations received.)

*Sale of products/services is not allowed. Give-aways/promotional/coupons are encouraged.
** Health Department regulations for food service must be followed. Call us if you intend to provide food samples.

Vendor Fee is $35.  The fees for this event help us cover the costs of table and chair rentals. You will be provided a 6-foot table and 2 chairs, unless you specify otherwise.  How will you be paying your fee?
