2021 European Tour Choir Agreement
Parent Guardian Permission *
I, the parent/guardian of the student listed below, hereby give permission for them  to participate in the 2021 Shakopee High School European Tour Choir. (TYPE STUDENT NAME BELOW.)
I will submit a $100 deposit to Mr. Kovic at the Shakopee High School by Tuesday, October 1st. (Click on box) *
I understand that if the student in question does not pass an audition, the full deposit will be refunded. All payments must be in check or money order form, and should be made out to: ‘SETC" (Shakopee European Tour Choir). Please write the student’s name on the memo line of the check. Handbook Policies & Guidelines (Click on box) *
I have read and understand the general guidelines, and all other policies listed in the '2021 Shakopee High School European Tour Choir Handbook' posted on the SHS Activities Website (www.shakopeesabers.com) and agree to abide by the contents therein.

I understand that the Tour Music Director and High School Administrator must have full authority to make judgments and decisions to ensure a safe and successful tour.

Payment Responsibility (Click on box) *
I understand that if the student cannot put forth a particular required cash payment by the date listed on the Payment Schedule listed in the handbook, the parent/guardian will be responsible for making that payment.

Tour Contingency Fund (Click on box) *
In the event that there is a medical emergency, or if a student needs to be sent home for health or other reasons, high school contingency funds will be used to cover the costs. If such an occurrence takes place, the student's family will immediately reimburse the amount spent on behalf of the student to Shakopee High School.

Rehearsal Responsibility (Click on box) *
I understand that it is the student’s responsibility to attend all rehearsals of the touring group, unless excused beforehand by the director. Participation in all Home Concerts as well as the ETC Farewell Concert is mandatory. Students will also be responsible for the performance of all concert music by memory for the tour. I understand that there is usually one morning and one evening rehearsal per week.

Consequences for Non-Compliance (Click on box) *
I understand that any student who is found to be in violation of Independent School District 720, Minnesota State High School League policies and regulations, or who demonstrates an unwillingness to accept the behavior guidelines stated in the Tour Handbook's Student Code of Conduct will be subject to the consequences set forth in the Consequences for Non-Compliance guidelines, also listed in the handbook.
