Registration Guidelines:
Only one (1) registration per form – complete one form for each classroom exchange.Only the teacher involved should complete this form.

Payment or purchase order must accompany this form
Make checks payable to “Artifact Box Exchange Network”. MUST BE U.S. FUNDS ONLY
Orders must be received by the deadline: (January 30)
You may also register online (extra $5 for credit card fees)
All spaces must be completed or this form will be returned to you

Please complete this form to register for the Artifact Box. You may register for one exchange per form. After we process your registration, we will e-mail you a confirmation contract to complete to verify your information and confirm your participation and agreement to follow the guidelines.

The cost for participation is $50.00 for an exchange, and $15.00 for a teachers guide.
