As per Canada Soccer, Ontario Soccer, the Niagra Soccer Association and other Governing Bodies, we as a Not-for-Profit Youth Organization are required to have all participants fill out these waivers on a session to session basis.  We have created this document to enable our members to have ease of use, to cut down on paper, and make it easier for us to keep track.  We ask that you complete this no later than up to 2 hours before attending your session so that we can check it prior to going on field/site.
