Service Project Form
As a component of Phi Alpha membership each student much complete two service projects prior to graduation (students who join during spring semester only have to complete one service project). These service projects were established as an integral part of Phi Alpha Honor Society philosophy.

This form is to provide a means of documentation of your service projects. Please hand in your completed forms as soon as possible to the Phi Alpha Student Assistant or the Phi Alpha mailbox in the mailroom on the second floor of Baker Hall (Room 207). Almost any community service activity and volunteer work will count towards fulfilling the requirement.
