Immunization Requirement for 7th Grade
Dear Parents/Guardians,

This letter is to inform you of important changes in the mandated immunization requirements affecting all incoming 7th grade students.

New vaccination laws now require all 7th grade students to have the following vaccines:

One (1) dose of Tdap – a vaccine which protects against whooping cough
One (1) dose of Meningococcal (MCV4) – a vaccine which protects against bacterial meningitis. A second dose of this vaccine will be required at 12th grade.

As with other school immunization requirements, students who do not provide documentation of these immunizations are subject to exclusion from school. Parents must provide the school with the documentation by the first day of the school year. This requirement applies to all SWL 7th grade students, both in-person and remote learners.

Please contact your child’s health care provider to make the necessary appointment to obtain these vaccines. If your child has received these vaccines, please submit the documentation to the school as soon as possible.

If your child does not have a doctor or if you are unable to obtain an appointment with your provider, the Licking County Health Department offers ongoing vaccination clinics with a variety of hours and locations. Please visit their website www.lickingcohealth.org or call at 740-349-6535 for more information regarding these clinics. You can also obtain these vaccines at local store-based clinics.

Records can be submitted directly to the school health clinic, faxed to 740-927-2337 or emailed to kparkinson@laca.org
