PAPER Gift Certificate Order Form
This order form is for a "Home Transformation Session" PAPER gift certificate to be sent via USPS mail.

To order a PAPER gift certificate to be sent in the mail for the above service, please fill out the form below and complete your payment via the secure checkout link at the end.  (Custom amounts are available; please contact me directly and I'll send you an invoice.)

Recipients will be able to redeem their certificate for either an in-home visit (Santa Cruz County residents only) or a virtual meeting (remote clients). Gift certificates do not expire.

A customized 5x7 paper certificate and envelope will be sent via USPS First Class Mail to the address you provide during checkout. It will be sent within 1-2 business days of purchase, with the choice of having it sent directly to you or the recipient.  Please be sure to enter all information carefully and double-check for accuracy.  

(Gift certificates are non-refundable, not redeemable for cash, and cannot be replaced if lost, stolen or misused.)
