NOTE: Prior to tryouts, all potential athletes will need the following completed and on file with our activities office:
1. A notarized and completed physical form
2. A paid activity card
3. A signed parental NSAA participation/waiver/consent form. (LOCATED ON THE BACK SIDE OF THE PHYSICAL FORM)
