Please note - This form is to request surrender.  This form gathers basic information about the reason for surrender and types of bird(s) requesting to surrender.  

We may not approve surrenders for a variety of reasons (limited/no available space, bird type not accepted, etc.).  Filling out this form does not guarantee surrender to this rescue will be approved.  We will do our best to contact you within 24-48 hours of this form being submitted to indicate whether we are able to approve the surrender request or not.  If you do not hear from us in this timeframe, please contact us through text, email, or our Facebook page to follow up.

Are there any health concerns/conditions with the bird(s)?  If so, please specify any information that will be helpful to Feathers Galore regarding their condition. *
Is the gender confirmed for your bird(s) - DNA testing, etc.?  Please provide any details that would be helpful regarding bird's gender, if known. *
What is the age of your bird(s)?  If more than one bird, please specify the name with their age.   *
How long have you had the bird? *
What history can you provide on the bird (previous owners, previous health issues, any known abuse/neglect, etc.)? *
How would you describe the bird's personality?  If more than one bird, please specify the name with the personality details.  This will help us in finding proper placement for the bird(s).   *
What is your bird's diet?  Seed, pellet, etc.  If a specific brand is used, please note this as well.  If any specific treats are preferred, please note these as well. *
Please provide any additional information about the bird(s) that may be helpful.  Does the bird have any preferences in regards to owner gender, cage placement in the home, bed time routines, etc.?  Please provide any information that will help the rescue and/or future adopter with the bird's care. *
If surrender is approved, how soon is the surrender wanted/needed? *
If surrender is not approved (or available), are you interested in other options to help rehome the bird?  (Example: If we are full or unable to accept the bird, we are sometimes able to post the birds on our Facebook page and help rehome without entering the rescue's care.) *
Are you able to transport the bird(s) to Feathers Galore?  Our address is 8655 S Preston Hwy, Lebanon Junction, KY 40150.  PLEASE NOTE: Visits, drop offs, etc to our location are by APPOINTMENT ONLY.  This is a private residence.  Appointments are required. *
