RFMS Drama "The Little Mermaid, JR" Sign-Up Form 21

This form is for students (and their parents) that are auditioning for the fall musical and/or interested in working behind the scenes as a crew member.
All new and returning Drama members need to pay the $25 fee. Send payment in an envelope or baggie with your student's first and last name on it to my classroom (610).
Please fill out the form in its entirety Auditions are Thursday, August 26, from all remaining-4:30, and a parent/guardian meeting is Monday, August 30 at 6PM.

I am responsible for attending ALL rehearsals and meetings, unless excused by Mrs. Tubb.

I am responsible for learning all lines and cues if I am a member of the cast.

I am responsible for learning all set cues if I am a member of the crew.

If I am not a member of the play, but I am a member of the Drama Club, I am responsible for supporting cast & crew members by attending at least one performance of the school play, as well as attending any Drama Club meetings.

I am responsible for maintaining a "C" average in all classes or I will be placed on academic probation.

I understand that becoming a member of the Drama Club does not automatically guarantee me a spot on the cast or crew.

I am aware that my student must pay a $25 fee (cash or check) in order to be a member of the Drama Club if I was not a Fall Drama member.

I am aware that if I owe the $25 fee, my student cannot audition without payment.

I am responsible for picking up my student on time from each rehearsal or meeting behind the cafeteria.

I understand that the tardy policy is in effect. If I am late to pick my student up 30 minutes+, my student will be dismissed from the show with no refund. If I am late 10-29 minutes late, my student will receive a strike, and two strikes will mean dismissal with no refund.

I understand that the absence policy is in effect. If my student has more than three unexcused rehearsals (no contact between directors and I), he/she will be dismissed from the program with no refund.

I am aware that I will be responsible for providing any costume or prop for my cast member (unless specified).

I understand that becoming a member of the Drama Club does not automatically guarantee my student a spot on the cast or crew.
