In order to engage my professional services, I ask all potential clients to complete the following enquiry form in detail so that I can get a better insight into you, your background and your goals.

Once completed I will review the details and endeavour to provide you with my Service Information PDF which details all of my services and associated costs within 3-5 working days.  

When you have selected the service level I will provide you with an invoice after which I will provide you with a pre-consultation form.  

After you have returned the completed pre-consultation form I will contact you to arrange a date for our consultation and I'll also provide you with details on how to complete a food diary and provide any other information ahead of our consultation.
