This form serves 3 purposes: 1) collection of contact info to keep you updated on our events, 2) a very brief survey to better understand you and what would bless you most in this season of your life and 3) a liability waiver for our insurance company so that you can participate in any future events without needing to sign a liability waiver every time.
Our insurance company requires that all participants at every event sign a liability waiver and photo release. This registration will be for all events hosted by St. John the Baptist Young Adult Community from May 2023-May 2024. Our group is targeted toward young adults ages 18-35.
