*My child is physically and mentally able to participate in this program. My child is fully toilet trained, able to self-feed and behaves in such a way that they do not pose harm to self or others.

*I will communicate my child's potential medical/medication needs with staff by calling Julie Brown/nurse at 360-335-3000 ext. 77189.

*I understand there is a non-refundable registration fee of $30/one child, $50/family due within a week of submitting this registration.

*I understand that monthly fees are due by the 10th of each month and are the same amount September through June. (one lump sum is divided equally by the 10 months of school). There are no reductions for holidays, illnesses or school closures. If you pay drop in fees rather than monthly fees, they will be added to your account.

*If fees are not paid by the 10th, I will be subject to a $25 late fee. If fees are not paid in full by the end of the month, my child's enrollment may be terminated and I will be subject to collections.

*I understand that I can change my child's permanent schedule twice during the school year at no fee. There will be a $20 fee each time I change my child's permanent schedule thereafter.

*I understand that my child can attend the days that I am signed up for. If I wish to add a day, I need to check with the Community Ed office to see if there is room.

*I understand that when I register for monthly childcare, I am paying to hold my child's spot, not necessarily for using it. For example, if my child misses a month I will still owe for that month to hold the spot.

*I understand that I must give the Community Ed office (360) 833-5547 a minimum 30-day notice if withdrawing from CED to not incur charges. A 30-day notice is also required if you want to go from full-time to Early Release only or drop in. A 30-day notice is also required if you want to go from full-time early release to drop in.

*I understand that Camas Community Education does not provide insurance. I, intending to be legally bound, waive and release my rights and claims for damages I may accrue against any and all sponsors of this program.

*We reserve the right to cancel enrollment for failure to adhere to any policy above.
