Attention parents please complete this quick survey if one of the following items apply to you and your family.
We plan to send food to every secondary student for a week which will cover Tuesday of the current week through Monday of the next week so that families continue to receive the benefit of the free lunch program. Please provide this information by 9:00 p.m., Monday, November 16. We will use the information provided today in this survey to compile a listing of those secondary students who will receive food deliveries.

Complete the survey if:
A.  You do not want to receive food for your students for the remainder of the week.  
      OR
B.  You need to have the food delivered somewhere other than your home address.

You only need to answer the question that pertains to your family.

All families who want the food delivery to their home address do not need to do anything but be available to receive the food when it is delivered by the TAT bus which we expect to be between 9:30 a.m. and 1:00 p.m. tomorrow.
