PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY

The following form may be used as part of the identification process for the MSAD 52 Gifted and Talented music program.

It is not necessary that all of the information on this form be filled out, however, an audition recording is required with this application (Please email to kara.getty@msad52.org).

An audition recording needs to meet the following criteria:

- Needs to be a video recording of the student performing.
Two separate pieces in contrasting genres.
