For the ad to be included in the program, you must do three things:
1. Fill out and submit this form
2. Submit payment via PayPal, search @FMHSTheatre
* Please put the student name and "Program Ad" in the memo line (+ What's this payment for?)
3. Submit text in the form below, email photos to fmhstheatrepublicity@gmail.com

We are happy to create the ad for you with your photo(s) and text. If you choose to create the ad yourself, please submit actual-size, camera-ready artwork, 300 dpi, black and white, PDF or JPEG. ******* Please submit ad content via email to fmhstheatrepublicity@gmail.com by December 1st, 2023.

If paying through PayPal, search @FMHSTheatre and please include your student's name and "PROGRAM ADS" in the PayPal memo line. This allows us to cross-check and verify all payments. ****** If you are not able to pay via PayPal, we will accept checks. Please mail payment to FMHS Theatre Booster Club, P.O. Box 270958, Flower Mound, TX 75027-0958. Payment must be received by December 1st, 2023.

Advertising Agreement "Signature" *

Your typed name below serves as a signature, indicating approval to publish the ad described above and submitted electronically. It also signifies understanding that if ad content is not submitted electronically by the deadline, it may not be included in the program. All ads must comply with the Lewisville Independent School District (LISD) policies and procedures.
