To opt out of FastBridge tests, the parent/guardian must complete this form. To best support the school district planning, please submit this form as soon as possible. 
  • A new refusal form is required each year by the parent/guardians who wishes to opt the student out of district wide assessments. 
  • A new form is required for each student in the family.
