Before your performer begins this musical theatre journey with us, we kindly request that you read and sign this liability waiver. This document aims to ensure the safety and well-being of all participants during the course of the program

Assumption of Risk: I, as the parent or legal guardian of the participating child, acknowledge that engaging in musical theatre activities involves certain inherent risks, including, but not limited to, physical exertion, dance movements, acting exercises, and vocal performances. I understand that while all reasonable precautions will be taken, accidents and injuries may still occur.
Release and Indemnification: In consideration of allowing my child to participate in the musical theatre class, I hereby release and hold harmless the organizers, instructors, venue owners, and staff members from any liability for any injury, loss, or damage that may arise as a result of my child's participation in the program.

Medical Treatment: In the event of an injury or medical emergency, I authorize the instructors and staff to seek medical treatment for my child and agree to assume all costs associated with such treatment.

Photography and Publicity: I grant permission for my child's photograph or video taken during the class to be used for promotional and publicity purposes related to the musical theatre class, including online platforms and printed materials.Untitled Title
Code of Conduct: I understand that my child is expected to adhere to the class's code of conduct, which includes respecting instructors, fellow participants, and the facilities, and refraining from disruptive or harmful behavior. Failure to comply with the code of conduct may result in the dismissal of my child from the program.

By signing this liability waiver, I acknowledge that I have read and understood its contents and voluntarily agree to its terms and conditions. I am aware that this document is a legal contract and that I am waiving certain rights and assuming certain responsibilities on behalf of my child. (please type name below)
