I acknowledge that I am responsible for my own health and physical condition. I am aware that my participation in the personal training session could cause injury and health risks. I understand that the coach, is not liable for any accidental injuries, illness or loss of personal belongings. I assume all risk connected to all programs and consent to participate.                                                              It is my responsibility to disclose any physical limitations, disabilities or related medical restrictions that may affect my ability to participate in the training session.                                                                                                                                                                                                                                                                                                          I will not hold Ultimate Athletics, or associated sponsors or venue responsible for any loss or damage of belongings,  injury or illness including Covid - 19  of the participant . We reserve the right to make any necessary changes to the program prior or during the activity.                                                                                                                                                                                            I understand no refunds will be made unless injury prevents training. Cancellation must give 24 hours notice or I will be charged for the session.
