You may complete this form anytime BEFORE the end of intermission for your show. You may remain anonymous if you like, but if you'd like to throw your name into the pot for a $50 gift card to Golden Valley Brewing Company, include your name. Winners need to be present to accept their card. 

Just like your average mad libs, fill in the blanks with adjectives, nouns, verbs, or whatever is asked. The funnier, the better!  The ensemble will act out your crazy suggestions in Act 2. Have fun!
