Volunteer as a ChIMES Guest Musician!
We are looking for SDYS students to demonstrate their instrument for young children ages 0-5 and their caregivers as a Guest Musician for SDYS' ChIMES program.

As a Guest Musician, you will be asked to perform a short piece (your choice, less than 2 minutes long), as well as a children's song that families can sing along to (e.g. Itsy Bitsy Spider, Twinkle Twinkle, etc). You can also talk about your instrument and answer any questions that families might have (e.g. "How long have you been taking lessons?", "What do you like best about your instrument?")

This is a fun experience that not only helps inspire and educate our youngest SDYS kids, but also counts for volunteer or community service hours!

LOCATIONS: Wednesday and Saturday ChIMES classes are held outdoors in Liberty Station; Thursday and Friday classes are held outdoors at the Marston House in Balboa Park. 

TIMING: The guest musician time slots are approximately 10 minutes long (out of a 45-minute class), starting at 9:45 a.m. and 10:45 a.m. on Wednesdays, Thursdays, and Fridays; and at 9:30 a.m., 10:30 a.m. and 11:30 a.m. on Saturdays. We hope you can volunteer for all the time slots on one day, but if this isn't possible, just let us know!

Guest musicians are welcome to stay and observe during classes, or walk around and explore the area in between volunteer times.

Please review the following important details before submitting this form.
**PLEASE NOTE:  Per State and County-Mandated COVID-19 protocols, the following procedures will be enforced with NO exceptions:

*The liability waiver at the end of this form must be signed (by checking the box) by the parent/guardian attending class with the student musician.  

*Before you leave home: Please do a quick self-assessment and stay home if you or anyone living in your home is showing any one of these symptoms: fever, cough, shortness of breath, difficulty breathing, other respiratory symptoms or any of these symptoms: chills, repeated shaking with chills, muscle pain, headache, sore throat or new loss of taste or smell. Also, please stay home if you or anyone you’ve been in close contact with has been exposed to COVID-19.
 
*Guest musicians are only expected to be on-site for 10-15 minutes, but you are welcome to stay for more of the class if you like! If you do, please bring a blanket to sit on, as well as sunscreen and water bottles if you wish.

*Please bring your own folding chair, music stand and music clips if needed.
SDYS Assumption of Risk and Waiver of Liability Relating to COVID-19
This waiver and declaration must be completed prior to participating in in-person classes, rehearsals, or events. This document covers all aspects of your attendance at, and/or participation in, programs organized or presented by the San Diego Youth Symphony and Conservatory (SDYS) in the 2021-2022 season.
 
1)  I acknowledge the highly contagious nature of COVID-19, and I voluntarily assume the risk that my child could be exposed to or infected by COVID-19 by participating in SDYS activities. Being exposed or infected by COVID-19 may lead to injury, illness, permanent disability, and death.
 
2)  I declare that my child is participating voluntarily in SDYS’ in-person activities.
 
3)  I agree that my child will (or I will help my child) perform a self-assessment for symptoms of exposure to COVID-19 (including fever, cough, sore throat, respiratory illness, difficulty breathing) prior to participating in any in-person activities.  My child and/or I will not participate in in-person SDYS activities if symptoms exist, or if anyone in my household has these symptoms.  Depending on the severity of symptoms, my child and/or I will attend virtual classes or rehearsals.
 
4)  My child and I agree to adhere to the requirements and recommendations of governmental authorities and to the safety regulations put in place by SDYS as pertaining to COVID-19.  This includes the following:
- Maintain physical distancing at all times.
- Wear a face mask that covers both nose and mouth at all times indoors (children under two do not need to wear a face covering).
- Wear/use personal protective equipment (PPE) for any wind/brass instrument.
- Follow procedures for entering and exiting classrooms/facilities safely.
- Comply with all posted signage.
- Not congregate in areas such as courtyards, bathrooms, doorways, or hallways before or after class.
- Outside of classes, observe local, city, and state policies regarding safe physical distancing, as well as public health advice from the CDC and state and local health departments.

5) My child and I agree that, by signing this waiver and agreeing to the terms and conditions set out herein, I waive all claims against SDYS, its officers, directors, and employees, the City of San Diego, the San Diego Park and Recreation Department, Liberty Station Community Association, and RiverRock Real Estate Group in the event that my child becomes infected with COVID-19 and becomes ill as a result and/or transmits the virus to another person.
