We will meet with individual appointments again this year. Please visit the URL Below to select a day and time you can come in. Each session takes about 15-20 minutes.  You will need to bring the following required documents to start this process; 1. KY (or State) Certified Birth Cert. 2. Proof of Residency in the form of an LG&E or Lou.
