In line with the implementation of the National COVID-19 Vaccination Program, Higher Education Institutions (HEIs) are requested  by the Commission on Higher Education to provide data on the COVID-19 vaccination of students.

This survey covers the Navotas Polytechnic College students currently enrolled in the second semester of  academic year 2021-2022.

The personal information we are collecting is limited and protected by the Data Privacy Act of 2012.
