This form is to be completed by applicants 18 years and younger for any position involving the supervision of minors. It is used to help Wake Chapel Christian Church provide a safe and secure environment for children who participate in our programs and ministries. This application contains some questions that are personal in nature. Please be assured that sensitive information is kept confidential.

Basic Expectations of Children's Ministry Volunteers

* SERVING IN ANY OF THESE AREAS SHOULD NOT REPLACE NOR DOMINATE A YOUTH'S REGULAR CHURCH INVOLVEMENT.

CHILDREN'S CHURCH (PreK) / JUNIOR CHURCH (K-2nd): Will assist adult leader with lessons and activities.

UPWARD: Opportunities available for scorekeeper, assistant coach, referee, halftime captain, and concession worker

VACATION BIBLE SCHOOL: Partner with adult to lead a group, music, meals, or craft; Must be committed to attending VBS for the entire week.

NURSERY: Open to female youth during worship service; must serve with an adult volunteer

CHILDREN'S MUSIC: Assist children's music ministry as needed by the director; must commit to consistent attendance for the duration of the program for the school year.
