Healthy Transitions Podcast Lived Experience Series

The Healthy Transitions Podcast gives Oregon youth between the ages of 16 and 25 a platform to share their lived experiences.

We’d love to give you a space to tell your story. Please fill out this form if you’re interested! There is no “criteria” for your experiences. Answer the questions in any way that feels true to you.

Please write in full sentences. In an effort to preserve anonymity, avoid using the names of any people involved in your story. If you’re not quite sure how to do so smoothly, you may use fake names. You’ll notice that some of the questions have other prompt questions below them. These are just there to help guide you. You’re not required to answer them.

Below, you will have the option to specify how you would like your story to be told. There is no obligation whatsoever to be on the podcast yourself. Please opt for whatever feels most comfortable. If you do choose to read your own story on the podcast, you will receive a stipend. Everyone who fills out the form will receive a stipend, regardless of whether they tell their story firsthand.

Due to limited time and capacity, not all stories will be featured on the podcast. Thank you for taking the time to share yours!
