If you are signing out during the school day, please use this form to alert the main office of your departure.

Come prepared, You will need the following:
           *Your 9 digit Student # (found on the top left of your timetable)
           * A text, email or a handwritten note from a parent or guardian  

The teacher WILL NOT be taking phone calls.  If you do not have a note, text or email from a parent, your teacher may ask you to meet with your VP.

Once you have signed out, please leave the building immediately.  
If you are returning be sure to  sign back in at the Main Office.
