Camp Calvary SH 1 - Bus Sign Up ONLY*
On  Monday, July 15th we will be going to Camp Calvary for a 5 day overnight camp for kids going into grades 10-12, and recently graduated Seniors.  We will leave Calvary Baptist Church on the bus Monday, July 15th at 8:30am and return on Friday, July 19th around 2pm.

I hereby give permission for my child to ride on the Calvary Baptist Church bus to Camp Calvary on Monday, July 15th, 2024 and return to Calvary Baptist Church on Friday, July 19th, 2024.  I hereby authorize Calvary Baptist Church (CBC) to secure any emergency medical treatments necessary and waive and release CBC and its directors, agents, and employees from any and all liability, claims and demand for any personal injuries, sickness, death, or damages of any kind sustained by my child, as well as any expenses of any nature whatsoever which arise out of or are related to my child's transportation to Camp Calvary.  I further understand that CBC does not provide health insurance coverage for my child. *
