1. I, as the parent/guardian of the player listed above, hereby give permission for him/her to participate in the sporting activities of the ONE10SPORT Football Academy ("the Academy").

2. I hereby indemnify and hold the Academy, its agents, representatives and educators harmless against any claim or demand arising from any illness contracted by, disease contracted by, virus contracted by, the death of or injury to my child or any loss of or damage to property, of whatsoever nature and howsoever sustained, including consequential loss, arising from or occasioned by my child's participation in any such sporting or extra-curricular activities delivered by ONE10SPORT or at the ONE10SPORT facility.

3. I agree that, if in the opinion of the Owner of the Academy or his delegated deputy an emergency has arisen, and medical treatment be deemed necessary for my child, the Owner of the Academy or his delegated deputy shall have the authority (which is hereby delegated to the extent such delegation may be required) to consent to such medical treatment, including surgical intervention, on my behalf.

4. I accept that all precautions will be taken to ensure the safety and welfare of my child and that I will be held responsible for the payment of medical and/or hospital accounts where applicable.

5. As far as I am aware my child is physically capable of participating in the said sporting or extracurricular activities and he/she is in good health. However, the persons responsible should please note the following: [Please state aspects that the coaching staff should be aware of, e.g. allergies, tendency towards abnormal bleeding, epilepsy, etc.]

6. Registrations are only confirmed on receipt of the completed Registration and Indemnity Form, accompanied by payment/s by the assigned due date/s.

7. The minimum sign-up policy at ONE10SPORT is one (1) term, as scheduled by ONE10SPORT. Should the participant not attend sessions for any reason/s, the full termly fees still apply and are payable by the assigned dates.

8. Participants are automatically enrolled from one (1) term to the next, unless the parent/guardian has submitted in writing, at least 7-days prior to the start date of the forthcoming term, that the player will be withdrawing from the Academy. No refunds will be given if cancellation of the participants enrollment is submitted within the 7-day period leading up to the start of the term or if cancellation is submitted during the term.

9. Should the player cancel their enrollment at the Academy, the joining fee will be payable again if the player wishes to rejoin the academy at a future date.

10. The participant and parents/guardians agree that they have read and understood the Health and Safety procedures and protocols as outlined in the "Project Restart" booklet. As a stakeholder, the participant and parents/guardians agree to adhere to and abide by these rules and regulations at all times.

11. Intimation of cancellation on the participant's part must be given in writing and must be received by ONE10SPORT at least 7 calendar days prior to the commencement of the term. No refund will be given if cancellation is submitted within the 7-day period leading up to the start date of the term.

12. Photographs and video footage may be taken for promotional purposes. By participating, parents/guardians hereby agree for ONE10SPORT to use those images for the purposes specified.
