By participating in any Euclid Parks and Recreation Activity you acknowledge and agree with the fact that you may be photographed or videotaped for the purpose of our website, program flyers, or picture displays.

Liability Release

By virtue of my participation in Euclid Parks and Recreation sponsored activities, I, the undersigned, for myself and my heirs, personal representatives, successors, and assigns, hereby release and hold harmless the City of Euclid and its Parks and Recreation Department from all claims and rights of action of any kind which the I may now have or which may accrue, arising out of my participation in Parks and Recreation sponsored activities, or arising out of the use of City or Parks and Recreation premises or equipment or property of any kind or nature, including exposure to Covid-19, including injuries, damage, and any losses which are presently known as well as those which are unknown but which may develop or be discovered in the future. I agree to assume any and all risks inherent in the particular Euclid Parks and Recreation sponsored activity in which I am participating.
