In order to participate in RTMS Basketball this year, you must fill out and submit this form, plus you must have a physical on file in the nurses office. Please also read over the athletics contract attached in the email. Physicals are good for 395 days. If you do not have one, please get it done as soon as possible!

Once this is filled out, your coach will send you an a group invite via schoology. From there that is where you will go for all communication as your coach will post everything there. Coaches may decide to have small group sessions based off of various factors, as well as large group sessions when possible.

By joining this does not guarantee you ever get to play in a game. If/when we are able to go in-person for basketball, everyone will be notified on how that will happen, as well as all procedures involved.
