Please list everyone in your family who would like to perform.
Remember, this includes grown-ups too! Parents, extended family, etc.

Please number your list of performers. Include ages for kids, and family designation for adults. Like:

PERFORMER 1: Elroy Jetson, 8
PERFORMER 2: Judy Jetson, 15
PERFORMER 3: George Jetson (Dad)

In the next section, you'll be able to specify preferences for each performer (i.e. singing solo or only with group, rehearsal timeslots, etc.)
