Curatorship Program Practicum Enrollment Form
This form should be used as a guide when having conversations with those involved with your practicum experience.  It is also used to document your practicum credits with the Curatorship Program and should be submitted for each practicum experience (it is likely you will submit this form two times over the course of your program).  

This form should be completed after the following have been identified for each practicum:
- Work Site (e.g., Eskenazi, IUMAA, Lily Library, University Archives, Wylie House, etc.)
- Workplace Supervisor
- Professional Responsibilities
- Academic Responsibilities
- Faculty Advisor (this may or may not be the faculty advisor serving as your mentor in the Curatorship Program)

Please submit this form at least one month before you register for the practicum credit.

If the department in which you enroll for course credit has its own practicum/fieldwork form, you may submit that paperwork in place of the Curatorship form to askcur8@indiana.edu.

What do you anticipate will be your weekly work schedule?  This includes time in/with a collection/gallery/library/museum as well as time with your faculty advisor on readings and/or meetings.
Practicum Course Numbers
When identifying a course number for your practicum, you should use the practicum course number scheduled by the academic department that most-closely aligns with the subject of your practicum (AADM, ARTH, FOLK, HIST, ILS, etc.).  Oftentimes, academic departments have forms similar to this you need to submit before you are approved to register for their practicum/field work/research courses.  If an academic department's form is extensive, we understand you might not want to submit a seemingly-redundant form.  In those cases, please submit the academic department's form to askcur8@indiana.edu or drop off a copy in the University Graduate School (Wells Library, Fifth Floor, East Tower).

When academic department course numbers are not available, or if you are unable to register for one for any reason, we have a Curatorship Program specific course number you can use: GRAD-U 595.  GRAD-U 595 is scheduled each semester, and registration requires instructor's consent.  The Curatorship Program staff director is listed as the instructor of record but is instructor of record in name only.  The staff director receives your course grade from your faculty advisor and updates the grade roster on behalf of each faculty advisor.

Credit hours requested for the course (Approximately 45 hours per semester are required per (1.0) credit.  Therefore, three credit hours = 9 hours work as described above/week average/15 weeks).

Professional Responsibilities
Professional responsibilities are determined by your workplace supervisor and may be articulated in a 'formal' internship experience/job description.

Hours/week (on average) spent in/with a collection, gallery, museum, etc.
Supervision Schedule (when will you meet with your workplace supervisor to receive feedback?)
Academic Responsibilities
Academic responsibilities are determined by your faculty advisor perhaps in conjunction with your workplace supervisor (in some cases, your faculty advisor and your workplace supervisor could be the same person).  

Depending on the number of hours worked relative to the academic credit desired, academic responsibilities may be minimal.

Faculty Advisor for this course (may/may not be your Curatorship Program faculty advisor/mentor).  The faculty advisor's role perhaps will be minor when your practicum hours are part of an internship or part-time job.

Written or Other Work (to be submitted to faculty advisor)
Meeting Schedule (when you will meet with your faculty advisor to discuss and review your readings, professional responsibilities, and anything else related to this course)
Hours/week (on average) spent on readings, meetings with faculty, written work, etc.
Next Steps
After submitting this form, it will be routed to the Curatorship Program and then distributed to the Curatorship Program faculty director via email.  It will be returned to your IU email if there are any questions or concerns, and you then need to update the form per the comments and re-submit.  If the faculty director has no questions or concerns, the form will be emailed to both the faculty advisor and workplace supervisor via email.  Each will be asked to affirm the noted Academic and Professional Responsibilities.  The form will be returned to you via email if either person has clarifying comments.

The final form will be filed with the Curatorship Program, and you will receive a copy of the approved practicum enrollment form via your IU email.

Be sure to register for the course listed on this form; submitting this form is not formal course enrollment.

Upon conclusion of the course:
- you are required to present your practicum experience to your peers.  This is an opportunity for you to develop your presentation skills as well as learn from others (all Curatorship Program students with a practicum or capstone in the same semester as you will also present their experiences).  The date/time of this presentation will be arranged early in the semester so you can note it in your schedule.
- your faculty advisor will confer with your workplace supervisor on your final grade and provide your final grade to the appropriate academic department or the Curatorship Program staff director.
