Free Tree Drive Thru Reservation Form - October 2021
Residents of Nassau County, FL are invited to receive up to two of our 3-gallon trees at one of our free drive-through Adopt-A-Tree events in October 2021. Simply complete the pre-reservation requirements and then use this form to reserve your trees and select your drive through pick up date and location.

To ensure that you have the essential knowledge to pick the right tree for your site, AND feel confident on how to plant it and manage it during the critical period of the first 3-6 months, we have provided two important educational resources from University of Florida Cooperative Extension (UF/IFAS):  1. Tree species Fact Sheets  2. Tree Planting 101 YouTube Video.  When you use these resources, you make note of the special validation code which must be entered on this free trees reservation form. Below you should insert the VALIDATION CODE that appears in both of the educational tools listed above. Reservations without the validation code will not be honored.
