Please complete the following information if you are trying out for the 2024 MCHS Boys' Lacrosse team. In the event you make a team, we will need this information for the season.
Tryouts will take place on January 16th at 4:00p.m. at Duncan Creek Park. REMINDER: Parents/players are required to have a Rank One account set up and all forms completed for their athlete prior to participating in tryouts. Players need to bring necessary equipment to tryouts. Directions for Rank One can be found on the Mill Creek High School athletics page.
After teams have been announced, a Mandatory Parent/Player Meeting will be held on January 17th at 7:00p.m. in the media center at MCHS.
The Player Fee for this year is $900.
The first payment of $400 is due at the Parent/Player Meeting. There will be an opportunity to offset the remaining $500 by selling raffle tickets and/or sponsorships. Raffle tickets will be given out at the meeting. You can choose to sell all of them, some of them, or none of them. However you choose to make your second payment of $500 is up to you. It will be due in mid-February, along with raffle stubs, in full - date TBD.

**Venmo will be another option to pay, they have implemented fees - TBA**
Please enter three DIFFERENT jersey # requests. Requested jersey #'s will be assigned based on seniority and available sizes.
***Optional Conditioning and agility testing will take place on January 9th at 4:00p.m. at Duncan Creek Park. Underclassmen, if you have uniforms from the fall, bring them on this day.
