This form is intended to collect information in the pre-planning stage for the 2020-2021 school term.  Later in the summer, you will be contacted with further steps to complete.  Your child will not be registered in school until forms that are given at a later date are complete and the following documents are collected;  birth certificate,
immunization form, social security card*,  and two proofs of residency.
*Identification numbers can be assigned if you cannot provide this information.
