Program Explanation

This agreement is made today between Lidia Fraser the Coach of the program and the participant specified at the end of this document. The program in which you are about to enrol in will include all of the following:

1. Personalised training programs, exercise routine and lifestyle recommendations will be provided to the client. These will be altered based on your current condition and goals. Training routines are updated as needed throughout our term based on what is optimal for your progression.

2. The Client acknowledges that the Client takes full responsibility for the Client’s well-being, including all decisions made during and after this program. They understand that participating in physical activity and exercise can carry a risk and that they accept all responsibility for that risk, though the trainer - Lidia Fraser, will not impose undue risk and will work to minimise any.

3. Availability to communication via text, email, and phone calls. Understand that in some rare instances responses may take up to 24 hours, with all queries answered.

4. Sessions must be paid in full at least 48 hours prior to the session, unless a session package has been purchased prior. You may wish to pay set up an automatic payment to L Fraser 12-3192-0539396-37 or pay per session.

5. Any changes in health status is to be informed to the exercise professional - Lidia Fraser, immediately. Due care will be undertaken by the exercise professional at all times. Failure to do so will absolve the trainer in any legal action taken by the client against the trainer.

6. The participant/client will agree to give full effort and attention to each session to maximise their time with the trainer.

7. Agreeing to this will entail a 12 week commitment to PT at the agreed frequency of sessions. If the client/participant decides to cancel their PT session agreement post 12 weeks, they must give at least 4 weeks’ notice. A minimum commitment of 1x session per week is to be upheld. Note that if any programs are given out complimentary as part of PT, they will be charged for ($85 - usual program cost) if PT is terminated within 12 weeks.

8. If a session cannot be attended, the client and trainer will negotiate to reschedule and not fully cancel the session in order to maintain progression. I.e planning an extra session in the following day(s)/week(s) if missing/skipping a session.

9. Cancellation / Rescheduling policy - please note that at least 24 hours notice is required to change a session without charge. The session fee will be incurred if changes are made within a day's notice - with some exceptions depending on circumstances. No shows also will not be refunded.

This includes a free first session, where by a no show or less than 24 hour cancellation or change to the session will be charged for at the usual rate. Note that the free session will not be lost or forfeited by the client in this case, rather it would be arranged to take place at an alternate time.

10. The trainer will keep the Client’s information private, and will not share the Client’s information to any third party unless compelled to by law, all according to the NZ Privacy Act 2020.

11. A strong commitment by both parties (the trainer and client) will be necessary to see desired results from Personal Training, and as the client, I agree to committing and investing in my health and fitness.

12. Note that for a free session or consult or package deal, exemption is made to the the clause regarding a 12 week commitment. However all other offers that a client uses are subject to the minimum term commitment. Any shorter term of coaching made with the use of an offer results in full price for the coaching used in the offer - with balance due at the termination of coaching. E.g first week half price deal.

13. By participating in PT sessions you therefore agree to these terms.
