Student Services Team (CEG, EAD and ALD)
This form is used for all referrals to any of the three branches of the student services team. If you have any concerns about any of your students, whether it be emotional, academic or linguistic, please fill in this form and we will outline an action plan in our SST meeting.

You will have feedback on the discussion within one week of referring the student and will be approached by the designated case manager from the leading department.

Este formulario se utiliza para todas las remisiones que se realicen a cualquiera de los tres departamentos que conforman el equipo  que brinda apoyo a los estudiantes.  Si tiene alguna preocupación con respecto a alguno de sus estudiantes, ya sea a nivel emocional, académico olingüístico, por favor complete ese formulario y nos encargaremos de crear un plan de acción en nuestra reunión de equipo.

Usted recibirá información después de una semana de haber remitido al estudiante y el caso será manejado por la persona indicada del departamento que lo deba liderar.
