WAIVER OF LIABILITY: I understand a physical examination is not required to run in the Farmington Phoenix 5k or 1 mile kid race and I do so at my own risk. If in doubt as to my physical condition, I will seek the advice of a competent physician and abide by his/her counsel. In consideration of the privilege of participating, and for other good and valuable consideration, I, intending to be legally bound, hereby for myself, my heirs, executors, administrators and assigns, waive and release any and all rights and claims I may have against participating sponsors including the Coaches, Race Directors, Farmington High, their agents, representatives, successors, or assigns for any and all injuries, ailments or other consequences suffered by me in the race. Participants under 18 years of age must have written consent of their parent or legal guardian. I have read this waiver carefully and understand it.
