If you are not satisfied with the response or feel uncomfortable talking with the service provider directly, complete this form to lodge a complaint with HaDSCO. The Health and Disability Services Complaints Office (HaDSCO) is an independent Statutory Authority offering a free, independent and impartial resolution service for complaints about health, disability and mental health services in Western Australia and the Indian Ocean Territories.
